Job Summary Acute Rehab Unit Registered Nurse (Night Shift) provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and or chronic illness. Working together with a multidisciplinary team, the Rehab nurse through careful assessment creates and implements a plan of care, and interventions to assist patients in adapting to an altered lifestyle, and to attain and maintain maximum function while providing a therapeutic environment for both the patients and their family members development. Schedule:12-hour shifts, 5pm-5:30am. Job Responsibilities The Staff Nurse, Acute inpatient Rehab services, is a Nurse that provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and or chronic illness. Create and implement a written plan of care that addresses the specific needs and goals of each individual patient, making referrals to other professional services as required. Modifies the plan of care based on reassessment of the patient including observed patient response to treatment/care and evaluation of patient's progress toward goal completion and delegates tasks to team members, as appropriate within their scope of practice. Observe, document, address, and implement planned interventions/care, communicating the plan to all members of the team through accurate and timely documentation in the electronic medical record and, when needed, through verbal reports. Documents accurate patient information in compliance with policies and regulatory requirements. Provide patient care through the performance of selective procedures that include but are not limited to IV starts and restarts, blood administration and phlebotomy tasks, answering call lights and serving as testing personnel under CLIA1988 regulations as they relate to point of care testing. Administer medications, accurately and timely according to physician orders and organizational policy and procedure. Maintains knowledge of medication actions and side effects, recognizing allergic reactions and documenting patients’ response to drugs. Successfully transition patients, communicating the most current plan of care, through walking rounds and providing accurate, timely report to incoming staff. Advise physicians, other disciplines, family/significant others of changes in patient's condition or abnormal findings. Communicate clearly and concisely. Provide clear, specific educational and discharge information to each patient and/or patient family member. May precept new employees and oversee other members of the care team. Constantly assess patient for adverse reactions to the anesthesia and respond appropriately. Educate each patient on the potential for reactions or side effects of anesthesia. Provide post-procedure discharge instructions to the patient and authorized patient representative that include physical, psychological, social, safety and educational and needs of the patient. Provide orientation and training of new employees as assigned. Function as testing personnel under CLIA1988 regulations as they relate to point of care testing as required. Job Summary The Surgical Nurse (RN) is responsible for caring for patients during surgical procedures. The RN Surgery assists the operating room staff during surgery and assists perianesthesia RN’s with care to patients recovering from a procedure. Job Responsibilities Provide patients with surgery expectations prior to transporting to operating room. Develop and implement the nursing care plan for surgical patients. Prepare and deliver medications to sterile field. Sterilize, confirm and mark incision sites. Set up the operating room for the procedure/patient, ensuring all tools and supplies are sterile prior to the procedure. While in a scrub role, assist the surgical team by donning sterile masks, gloves and gowns as well as aid the surgeon by passing instruments during the procedure. Apply sterile techniques in handling medical instruments and equipment pre-, during, and post-surgery. Use and troubleshoot medical equipment safely and effectively to carry out planned nursing interventions, assist staff and obtain equipment needed. Post procedure, clear tools, prepare the patient for transport to recovery, and provide report to the recovery RN. Assist with the ordering, maintenance, and safe storage of surgical equipment and supplies. Perform other duties inside the surgical room that ensure safe and effective care. Accurately and timely document patient care to ensure proper post-procedure follow up and billing of services. Provide continued education on unit as needed and assist with orientation and training of new employees. Serve on call for rotating weekend, holiday and weeknights as needed. Develop and manage the department schedule and on-call schedule and provide call coverage in the rotation. Manage inventory and ensure the availability of materials, supplies and equipment for scheduled or add on procedures. Serve as a preceptor for new hires or transfers to the department and provide job specific training as assigned.
