Position Overview
The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population.
Primary Responsibilities
Clinical Supervision
Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts.
Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans.
Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications.
Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting.
Oversee medication administration and treatments to ensure safety and compliance.
Staff Leadership & Management
Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice.
Provide mentorship, education, and on-the-job training for new or less experienced staff.
Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator.
Promote teamwork, professionalism, and accountability across the care team.
Regulatory Compliance
Ensure nursing practices comply with New York State DOH, CMS, and facility policies.
Conduct chart audits to confirm accurate, complete documentation of resident care and progress notes.
Participate in QA/QI programs, infection control audits, and safety committees.
Prepare for and participate in DOH inspections and surveys.
Maintain resident confidentiality and HIPAA compliance at all times.
Emergency Response
Serve as the on-site clinical authority during assigned shifts.
Respond to medical emergencies, perform immediate assessments, and direct staff response.
Communicate critical incidents to the DON or Administrator on call.
Resident & Family Relations
Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld.
Communicate effectively with residents and families regarding care issues or changes in condition.
Participate in care plan meetings when required.
Qualifications
Licensure: Current, unencumbered Registered Nurse (RN) license in New York State.
Experience:
Minimum 2–3 years of RN experience, with at least 1 year in a long-term care or skilled nursing facility.
Prior supervisory experience strongly preferred.
Education: Graduate of an accredited nursing program (BSN preferred).
Certifications: BLS required; ACLS preferred.
Knowledge:
Comprehensive understanding of NYS DOH and CMS nursing home regulations.
Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols.
Skills:
Strong clinical assessment and problem-solving ability.
Excellent leadership, delegation, and interpersonal communication skills.
Ability to handle emergencies calmly and professionally.
Proficiency in electronic health records (e.g., PointClickCare, SigmaCare).
Work Conditions
Work performed in a skilled nursing facility environment.
Requires standing and walking for extended periods and occasional lifting up to 25 lbs.
May require evening, night, weekend, or holiday shifts.
Must be able to travel between Bronx facilities if needed.
