Discharge Planning with patient and caregiver education for a safe transition home is an essential part of the role. Admission to Discharge, you are an important member of the Interdisciplinary Team. The nurse will provide direct care for all new admissions, review transfer documentation, review and verify physician's orders, and ensure a safe and successful admission process.
