 Key Responsibilities

  • Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.
  • Provide high-quality nursing care in alignment with the patient’s care plan, including comprehensive health and psychosocial assessments to support recovery.
  • Monitor patients’ conditions and coordinate health promotion, prevention, and direct care.
  • Maintain detailed and accurate clinical records, including all required OASIS documentation.
  • Communicate effectively with clinical managers and colleagues to ensure high-quality care.
  • Provide training and support to patients and their families/caregivers.
  • Function as a key point of contact between patients and community health resources.
  • Work alongside physicians and discharge planners to provide health education to patients and caregivers. 
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
  • Understand and adhere to the agency’s process for managing and addressing customer complaints.
  • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
  • Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.  

Qualifications

  • Must have a current RN license (state-specific). 
  • Valid BLS certification. 
  • Minimum one year of experience in home health or a healthcare facility. 
  • Familiarity with Medicare and Medicaid guidelines. 
  • Possess a solid understanding of professional nursing principles and home health documentation. 
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

We are seeking a Telephonic Nurse Case Manager with compact and CA RN license to work Monday - Friday, 8 AM to 5 PM. Bilingual (Spanish speaking) is strongly preferred. Candidates with additional RN licensure from CT, IL, MA, and NV, and candidates with NCIC Registry and QRP (GA Rehab Supplier) will also be shown preference. The Telephonic Case Manager provides remote medical case management services to injured individuals, many of whom were industrially injured. A Telephonic Case Manager assists consumers in their recovery so that they may return to the highest level of function as possible. Medical services are coordinated and assessed frequently, vocational options are explored with the injured worker and their employers, and pre-injury employment or alternate employment is secured. The Telephonic Case Manager works with all parties to the claim including: the injured person, the claim’s examiner, employer, attorneys (plaintiff and defense) and medical providers.
