Summary GENERAL SUMMARY The LPN - Urgent Care reports to department leadership and is responsible for providing clinical care and services in close cooperation with and under the clinical guidance/supervision of the practice provider(s) and/or registered nurse. Performs a wide variety of patient care activities, accommodative services, and administrative functions to support the medical care of patients and families. ESSENTIAL DUTIES AND RESPONSIBILITIES Assists physician, NP and/or PA in all aspects of medical care, treatments and procedures, diagnostic and laboratory testing following established policies, procedures, and regulations. Participates in on-going quality assurance, maintains regulatory requirements, and assists in establishing and updating applicable policies and procedures. Provides support to orientation and training of staff and students. Demonstrates excellence in nursing practice characterized by quality patient care and participation in formal and informal patient, family and staff education. Demonstrates effective use of time and resources. Provides coverage as needed to locations outside of primary work location based on operational needs. Performs other duties as assigned. LICENSES AND/OR CERTIFICATIONS Current State license as a Licensed Practical Nurse. CPR and PEARS Training required and/or must be obtained within 45 days of hire. PALS certification required or must be obtained within 6 months of hire. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS Professional knowledge of nursing theory and practice. Pediatric experience preferred. Ability to travel. Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing. WORKING CONDITIONS Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment. Newport News, Virginia Overview The Licensed Practical Nurse (LPN) will perform duties under the direction or supervision of a licensed medical practitioner or a registered nurse. The LPN is responsible for conducting a focused assessment of the patient to include vital signs and gathering data, implementing established plans of care, and evaluating effectiveness of treatments in the medical practice setting. Position requires rotating to all urgent care locations. What you will do Accurately completes all required clinical documentation consistently per RHS standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible and completed same day. Knowledgeable and skillful in lab functions. Monitors OSHA and lab proficiency testing. Maintains clean, compliant, well-stocked work areas. Provides patient education as needed. Teaches patients as necessary about the medications prescribed for them. Demonstrates positive patient care by explaining the importance of the completion of registration and patient medical history forms and how it relates to their care at our facility. Provides patient follow up with a sense of urgency regarding lab calls, patient phone calls, prescription fills and refills, referrals and appointment scheduling. Answer telephone calls from patients, pharmacies, physicians, vendors, etc. by close of business. Communicate lab/pathology results and schedules additional tests per provider order. Ensures proper reimbursement from insurance companies by performing pre-authorization for prescription drugs, procedures, and patient tests. Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings as assigned.
