We're looking for compassionate caregivers to join our team and assist clients with their daily living activities. Following personalized care plans established by our agency Care Managers, you'll provide a range of support, from companionship and meal preparation to light housekeeping and errands. Depending on the client's needs, the role may involve transportation and personal care assistance, such as bathing and dressing.

Minimum Qualifications:

  • A minimum of 1,000 hours of experience working with geriatric or chronically ill individuals is preferred. This translates to approximately 6 months of full-time work or 12 months of part-time work. Exceptions to this requirement are made for recently licensed Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), or individuals who will only provide companionship and housekeeping services.
  • A CNA or HHA license is preferred but not mandatory.
  • Proof of current TB testing is required.
  • You must be physically able to perform the duties of this position, which may include transferring clients using proper safety techniques and equipment.
  • Reliable transportation with a valid driver's license and automobile insurance is necessary if you will transport clients.
  • Excellent communication skills are essential for effectively interacting with clients, care managers, and other team members.
  • A mature, caring, punctual, and reliable demeanor is critical to building client trust and rapport.
  • You must be able to pass a criminal background check (national and any required by state).
  • Verifiable professional and personal references are required.

Essential Duties and Responsibilities:

  • Understanding and Implementing Care Plans: You'll thoroughly review and understand each client's personalized care plan, ensuring you provide the necessary assistance as outlined.
  • Comprehensive Client Support: Your duties will vary based on client needs but may include companionship, meal preparation, and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation (walking), positioning, transferring clients safely, toileting, and providing protective supervision.
  • Client Communication and Advocacy: You'll notify the Care Manager of any client concerns, changes in care or client condition, and client emergencies. This ensures timely intervention and a high level of care.
  • Safety and Compliance: Following company Infection Control practices is essential. You'll also need to notify the office if you require Personal Protective Equipment (PPE) or additional supplies to perform your duties safely.
  • Timekeeping and Documentation: ClearCare Telephony Instructions will guide you in clocking in and out for all scheduled shifts. You'll also need to accurately update your task status for each client before leaving their home. For non-client-specific hours (like training time), you'll submit a Time Log and Time Certification for Non-Visit Time form.
  • Professional Appearance and Conduct: Employees are expected to maintain a professional demeanor and adhere to the company dress code as outlined in the Employee Handbook.
  • Dependability and Teamwork: Reporting to work on time and being ready to work is crucial. You'll also participate in ongoing training to stay updated on client care needs and company policies. Regular staff meetings are essential for collaboration and information sharing. Finally, adhering to all company policies and procedures, as detailed in the Employee Handbook, ensures a smooth and efficient work environment.

Organizational Relationship:

The caregiver reports directly to the Care Manager for guidance on client care and receives scheduling direction from the Staffing Coordinator.

Physical and Environmental Demands:

The role requires the physical ability to reposition and transfer clients using proper techniques and equipment safely. Travel to client residences using reliable transportation is also necessary.
