We’re seeking a caring, motivated LPN or RN who loves connecting with people, coaching caregivers, and helping seniors thrive safely in their homes. This is a hands-on, field-based role that blends nursing knowledge with relationship building and training. What You’ll Do Conduct client assessments and create or update individualized care plans. Provide training, coaching, and delegation for caregivers in the field. Collaborate with families and the care team to ensure high-quality, compassionate service. Support medication oversight and related documentation as needed. Communicate effectively with caregivers, clients, and office staff to keep care seamless. Serve as a trusted nursing resource for caregivers and clients.
