Psychiatric Nurse Practitioner - PMHNP

Part-Time Schedule: 2-days per week (excluding Wednesday) at estimated 6hrs/day - 9:30a - 3:30pm

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience.

As a Psychiatric Nurse Practitioner (PMNHP) you are an integral part of developing and delivering a personal care plan, ensuring high quality, clinically appropriate, team-based behavioral health care. Physical attendance at an InnovAge center(s) is essential to performing the job duties with the option to travel to meet participants outside of center. No on-call or weekend hours required. 

You will 

  • Confidently and swiftly navigate ambiguous situations to manage participants’ care in real time 
  • Take bold action using evidenced-based medication management to support acute needs
  • Emphasize gradual dose reduction
  • Natural ability to integrate with primary care and other Interdisciplinary Team (IDT) members
  • Adapt interpersonally to meet participant and IDT clinical needs in real time 
  • Pioneer InnovAge’s developing behavioral health integration program
  • Serve as mentor/educator to PCP’s regarding psychotropic medication management and behavioral change

Clinical Care – 70%

  • Provide in-person clinical assessments of participants, prescribe and deprescribe, identify treatment protocols, and on-going medication management in centers and via telehealth
  • Coordinate care with Behavioral Health Providers, PCPs, and IDT
  • Consult with IDT staff on behavioral interventions, nursing practice, and psychopharmacology, including writing orders for medications, as appropriate
  • Provide integrated therapeutic care 
  • Monitor psychiatric hospitalization care and follow-up disposition planning
  • Respond to urgent calls from clinics, center staff, and facilities, with a solution-focused approach
  • Monitor response to psychotropic medications

Administration – 30%

  • Educates IDT to manage and resolve behavioral complexities or placements
  • Liaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring/follow up and report to IDT 
  • Provide in-service education at PACE centers; participate in staff meetings as needed
  • Contribute to case review, consultation, and utilization review
  • Research and utilize evolving psychiatric treatments, PACE service requirements, and legislation
  • Participates in IDT meetings, conferences, committees, and events as requested

REQUIRED

  • Current State issued Psychiatric Nurse Practitioner/PMHNP license
  • 2+ yrs in psychiatric care and medication management
  • 5+ yrs nursing with at least 2yrs as Nurse Practitioner
  • DEA Certification with full prescriptive authority
  • Proficiency with standard Microsoft Office applications and EMR systems
  • Current CPR certification or the ability to obtain CPR certification in first 30 days of employment
  • Requires personal transportation, valid state issued driver’s license, good driving record and auto insurance as required by law.

PREFERRED 

  • Experience working with geropsychiatric populations
  • Licensure or certification in substance use treatment
  • Training or experience with group and family interventions
  • Experience with conflict management, mediation, de-escalation, and crisis mitigation
  • Knowledge of the chronic disease management and psycho-social implications
  • Bilingual

Nurse Practitioner

Monday through Friday 8 to 5pm, no nights, weekends, or holidays! The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.
