Licensed Practical Nurse - Women's Health

As a Licensed Practical Nurse (LPN) for Swope Health Services, you will be part of a team of caregivers to provide care for the people who need it most. As an LPN you are responsible for taking a health history, performing health counseling, and administering medications and treatments. You will provide nursing care and assist our physicians and providers. LPNs provide follow-up on abnormal lab results, abnormal imaging results, and post-hospitalization, as well as proactive pre-visit planning, and patient teaching. You are a critical part of our patient-centered medical team that provides quality care. Contacts patients regarding abnormal labs and imaging results to advise patient of results and further care. 2. Transcribes physician/provider orders, collect specimens and cultures for laboratory analysis. 3. Reviews telephone encounters and addresses within the LPN scope of practice. 4. Completes forms and documents and obtains physician signature as appropriate. This includes but not limited to diabetic supply form, DME, and Home Health Orders. 5. Arranges referrals for specialty care, urgent or emergent care at the direction of the provider. Instructs patients in home self-care when an immediate clinic evaluation is not necessary. Instruct patients to come to the clinic as necessary. Consult with the clinic manager or provider when there is a question about the severity of the problem or the need for immediate attention. 6. Administers oral and parenteral (IM, subcutaneous, intradermal) medications as ordered and prescribed by the provider using the 6 Rights of Medication Administration. May dispense non-prescription sample medications kept in the clinic only. May not dispense prescription sample medications 7. Performs waive testing and venipuncture as ordered by the provider 8. Documents all nursing care and treatments performed, in the electronic medical record. 9. Coordinates pre-visit planning by reviewing the electronic medical record to include patient history and proactively orders labs and other testing based on standing orders. 10. Performs hospital follow up by reviewing discharge summary and performing med reconciliation. 11. Follow up on any referrals. 12. Delegates to MA within scope of practice to enhance patient care. 13. Manages and directs patient flow within the clinical area. 14. May relieve the nurse supervisor and medical assistants as necessary. 15. Orders basic clinic supplies and stock medications. 16. Knowledgeable on using PC and Microsoft Word. Ability to document within an electronic medical record and/or familiarity with Windows - based programs. 17. Assists in staff training and new employee orientation. 18. Maintains an environment conducive to the care and welfare of patients/clients. 19. Participates in continuing educational offerings that are scheduled at the facility. 20. Comply with computer security guidelines and confidentiality of both patients and staff. 21. In an emergency, L.P.N. may summon EMS for transport, initiate CPR and take actions necessary to preserve life. 22. Checks the oxygen and respiratory area weekly. Documents amount of oxygen on log sheet. Stocks room with appropriate supplies and equipment. 23. All other duties as assigned by the clinic manager. Successful Candidates will have: Graduated from an accredited school of practical nursing. Licensed to practice in the State of Missouri and/or Kansas. Basic Cardiovascular Life Support (BCLS), certified. Excellent communication skills, customer service oriented and exhibit the ability to work with people (staff, visitors and clients). Primary Functions: Conduct health assessments on pediatric patients with both acute and chronic medical conditions. Develop an impression of diagnosis and a treatment plan in collaboration with the physician. Conduct health education/counseling/prevention of individual patients/groups at Swope Health Services. Participate in interdisciplinary patient staffing. Coordinate/collaborate with Swope Health Services departments and community service organizations as required by funding agencies. Responsible for integrating healthcare and health education in the Continuum of Care process. Order/perform and interpret routine laboratory tests. Develop a health promotion and maintenance plan, including disease prevention, health education, and counseling. Provide care for selected complicated patients as directed and supervised by department physician(s). Arrange referrals as needed to other members of the health care team in accordance with SHS policies/procedures and/or third-party payers Successful Candidates will have: Master of Science in Nursing or a Doctoral Degree Program 2-3 years experience in Pediatric setting preferred. Registered Nurse in the State of Missouri and Kansas with an ARNP License. Completion of a recognized nurse practitioner program in Pediatrics or Family Medicine in a university-based or university-affiliated setting. Ability to work with a culturally diverse population. Ability to function as part of a team. Licensure through the National Council Licensure Examination.
