Responsibilities The RN Case Manager is responsible for providing support and clinical expertise across the continuum of care, from admission through discharge or transition to the next level of care. Collaborates with physicians and the interdisciplinary team to coordinate patient care, enhance clinical outcomes, support patient and family engagement, and manage the cost and efficiency of care delivery. This role integrates utilization management, care coordination, and discharge or transition planning functions. As part of our Level I Trauma Center, the largest of three acute care hospitals, you will be key to the continuum of care of Valley patients and beyond! In this 30-bed unit with patient ratios of 1:2, you will work closely with fellow team members, including our Neurological Intensivists, who provide teaching in an individualized or group setting.
