Key Responsibilities Collaborate with a multidisciplinary team to address the unmet needs of patients. Conduct telephonic outreach and home/virtual visits based on patient acuity. Perform comprehensive health risk assessments and disease management evaluations. Provide outreach, education, and interventions for patients with high utilization patterns. Develop patient-centered care plans in collaboration with patients and care teams. Coordinate services to optimize clinical outcomes and track patient progress. Identify and address barriers to treatment, facilitating resources and referrals. Build collaborative patient relationships through education on health preservation and self-management. Participate in regular caseload reviews and consultations with the care team. Assist in securing medical supplies for patients as needed. Maintain meticulous medical documentation and adhere to care management standards. Contribute to the development of new care standards and participate in staff meetings and educational offerings. Minimum Qualifications • Registered Nurse with an active, unrestricted Texas licensure. • 3-5 years RN experience in an acute care setting; other clinical settings may be considered. • 1-2 years case/care management experience. • Experience with all patient populations, including Commercial, Medicaid & Medicare. • Computer literacy with ability to operate computer programs and documentation platforms. • Strong customer service skills for service delivery and resolution of issues. • Discretion and secure information management. • Desirable: Case Management Certification, Motivational interviewing, Federally Qualified Health Clinic (FQHC) experience, Bilingual (Spanish), Ability to think critically and problem solve.
