Responsibilities In collaboration with the physician and your preceptor, you will ensure implementation of the assessment plan, evaluation, and follow-up of the plan of care for the patient population. Initiating an ongoing systematic assessment of the physical, emotional, social, educational and functional needs of the patients/families. Interpreting assessment data and information to develop a nursing care plan which contains problem identification, nursing diagnosis, nursing orders, approaches, interventions, short/long term goals and discharge planning. Participating as a productive member of the interdisciplinary team in assessment, planning, implementation and evaluation of rehabilitation goals. Assuring that documents in the medical record are complete, factual, accurate and timely. Qualifications Minimum requirements: Graduate of an accredited program for registered nursing. We specialize in intensive rehab for stroke recovery, traumatic brain and spinal cord injuries, orthopedic injuries, and other complex conditions. Our compassionate, multidisciplinary team provides comprehensive physical, occupational, and speech therapy in a supportive, healing environment. Joint Commission-accredited and certified in stroke and brain injury rehabilitation, Summa Rehab Hospital is nationally recognized (including being named one of America’s Best Rehab Hospitals by Newsweek) for delivering patient-centered care that helps individuals regain independence. What you bring: A passion for providing patient care, strong clinical skills, and a strong collaborator with Physicians and Therapists. Degree from an accredited Nursing Program. Current State Registered Nurse License. Current CPR Qualifications Required Skills: Successful completion of accredited Registered Nursing program required. Bachelor of Science in Nursing degree preferred. Current state license as a Registered Nurse required. Current CPR Certification required. One (1) year recent experience preferred. Acute care setting preferred. ACLS certification preferred in LTACH and Texas hospitals.
