This role supports MultiCare’s Bed Readiness Program through system‑wide referral coordination, placement, and active case management of patients in the INW and Yakima markets. The position requires an in‑person Case Manager serving the Yakima or Inland Northwest geographic areas. FTE: 1.0, Shift: Day, Schedule: Monday - Friday Position Summary The Hospital Case Manager RN is responsible for high quality cross continuum care coordination. This individual works with physicians/APP, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care and community engagement. Responsibilities You will ensure that the patient and family experiences medically appropriate transitions of care, and an integral and essential component of the care delivery process You will advocate for patients and families, including transition to lower level of care when inpatient care is no longer needed You will partner with and supports physicians/APP in patient care activities and care transitions You will utilize critical thinking in all interactions You will be accountable for transitions of care and care across the continuum Requirements Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred Current Registered Nurse license in Washington State or Multistate License endorsement (MLS) Four (4) years recent hospital case management experience Five (5) years of recent clinical hospital/ambulatory experience Healthcare financial and reimbursement knowledge preferred Leadership and emergency services experience a plus Consideration may be given to internal candidates not meeting the minimum qualifications A Better Way to Work in Health Care Flexible schedule: Make a positive difference in your community and have time and energy to pursue your passions outside of work. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. CME and license fee reimbursement —we've got you covered. * Offered benefits dependent upon employee eligibility and criteria Job Description: Purpose of Job: Improve the overall health of the communities we serve by providing clinical leadership, oversight, and development for clinic‑based Registered Nurses to ensure safe, high‑quality, and top‑of‑license patient care as follows: Essential Duties and Responsibilities: Develops and oversees clinic-based nursing on-boarding and ongoing training. Responsible for clinical oversight of clinic-based RN’s including assuring relevant clinical knowledge, physical assessment skills, and appropriate use and understanding of CHAS policies and procedures. Responsible for sharing RN clinical performance with Clinic Administrators during evaluations. Creates and maintains guidelines, workflows, encounter plans, and order sets pertaining to the day-to-day activities of clinic-based RN’s, to ensure top of license work. Write, in conjunction with Director of Nursing, policies, procedures, and standing orders related to the roles of the clinic-based nurses. Collaborates with appropriate teams to design, implement and oversee workflows relating to quality measures and population health, ensuring top-of-license work. Works with Director of Nursing, Quality Improvement Specialist, and Director of Operations to monitor clinic-based RN peer review process and quality dashboards. Maintains clinical skills by functioning as clinic-based RN at least one day a week. Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values. Qualifications: Education/Experience: Minimum of bachelor’s degree in nursing required. Valid RN license in the state of Washington or Idaho required upon hire. Nursing experience, preferably within a community health setting required. Experience with case management preferred. Experience with electronic health records preferred. Skills: Excellent customer service skills required. Strong oral and written communications skills required. Compassion for patients required. Ability to work well in team environment is required. Ability to multi-task in fast-paced work environment required. Ability to anticipate future needs required. Computer skills required. BLS (CPR/AED) required. Valid drivers’ license and insurance required. Commitment to supporting a safe and respectful environment is required. Physical Demands Must be able to move around the facility between one-third and two-thirds of the day, as well as sit at a workstation and in patient rooms. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Communicating is also required over two-thirds of the day, while climbing, stooping, or crawling is minimal. Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. occurs only occasionally.
