Assists LIP with procedures, removes sutures, cleans wounds, applies dressings. References policies and procedures to assure Capital Health best practices are followed. Recognizes important changes in patient condition and reacts appropriately in the Ambulatory setting. Administers medications accurately and safely. Documents patient's records according to established protocol. Performs and demonstrates technical skills specific to assigned Ambulatory setting. Administers immunizations, and other injections as ordered by LIP according to established protocol and documents. Documents quality control for waived testing and sterilization of instruments as required according to established policies and procedures. Performs Quality Assurance activities as defined by Ambulatory setting. Maintains logs of laboratory work, X-rays and other tests and monitors receipt of results to assure continuity or care. Documents refrigerator temperatures for proper storage of medications and specimens and takes appropriate action if outside acceptable range. Triages patient medical phone calls, assessing the patients' status, and schedules according to established protocol. Performs and documents part of the physical assessment including, but not limited to chief complaint and vital signs. Performs other duties as assigned and may be assigned to other Capital Health Medical Group Practices as needed. MINIMUM REQUIREMENTS Associates degree or graduation from an accredited school of nursing. One year previous medical office RN experience. Towne Home Care is hiring for an RN Supervisor to join our growing team! The RN Supervisor will assess new and incoming home care clients, as well as supervise and educate field staff on clinical items and care needs, and act as a liaison between the office and clients. Swan Hospice is actively seeking Registered Nurses (RN) to join our team in Central New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Job Description A. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems of the patient at home Works closely with other members of the IDT to ensure all patient and family care needs are being met. Provides supervision to Hospice Aides/Homemakers. Education a. Graduate of an accredited registered nursing program. b. Bachelor’s degree in Nursing preferred. c. Certified Hospice and Palliative Nurse (CHPN) certification preferred. 2. Experience a. Has minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience. b. Overview Pediatric emergency department nurses are specially trained to care for children and adolescents. Children are fast-tracked to a child-friendly space separate from the adult care areas. This space includes private rooms decorated in child-friendly themes, all designed to create a comforting environment to put an apprehensive child at ease. Responsibilities Provides considerate respectful care focused upon the patient's individual needs to ensure patient satisfaction. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the patients cared for. Demonstrates knowledge of growth, development and/or adult health throughout the life span as appropriate to the job performed. Successfully completes life-span education as appropriate. Obtains a complete nursing history and baseline assessment that is systematic, consistent and continuous. This assessment is communicated effectively utilizing Hand-off policy and documented. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in medical record according to hospital/nursing policy and the Nursing Standards of Care/Practice for the specific patient type. Appropriately analyzes patient information /assessment to determine nursing diagnosis and needs for appropriate nursing care, medical intervention or services of other clinical professionals. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi-disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medication as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Participates in unit conferences to discuss quality issues and quality improvement strategies. Delegates components of care to non-professional caregivers as appropriate. Follows up on any delegated components of care and assignments given. Make decisions that demonstrate knowledge of nursing practice; utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the best patient outcomes. Follow CSHS Pain Assessment policy. Can successfully function as a Resource Nurse, making decisions in the absence of the Clinical Leader and/or the Nurse Manager. Supports cost containment through efficient use of equipment, supplies and other pertinent resources. Recognizes adverse reaction to medications and takes immediate and appropriate action. Evaluates the patient's needs for discharge and accesses appropriate resources for the patient and/or family. Consistently provides written and verbal education to patients and/or families. Can successfully function as a preceptor/mentor to students and new staff members. Consistently promotes teamwork within the unit and between departments. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy #4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Qualifications Education: Bachelor’s Degree in Nursing preferred. Work Experience: Minimum two years pediatric ED experience or experience in ED that cares for pediatrics requried. Licenses and Certifications: RN license/NJ required. BLS certification required. ACLS certification to be achieved within First Year then maintained thereafter. Successful Completion of Basic Dysrhythmia course to be achieved with First Year. CSMC IV Certification to be achieved within First Year. Stroke Education requirements to be achieved within First Year then maintained thereafter. Specialty professional nursing certification preferred. Non-violent Crisis Intervention certification -- New hires must receive certification within 3 months of hire. Renewal certifications will be completed every 2 years. How you make a difference The Charge Registered Nurse is responsible for overseeing the total health care delivery system in their assigned nursing unit, including the collection and evaluation of health status data, nursing diagnosis, goal setting, planning, implementation, and evaluation of patient care. Maintaining a professional atmosphere and attend monthly staff meetings as directed by the Director of Nursing or Administrator.
