Why M Physicians? The Pediatric Discovery Clinic offers a wide range of services to pediatric patients seeking care from any of our 15 sub-specialties. Some of the pediatric sub-specialties we see include; surgery, dermatology, endocrine/diabetes, nephrology, urology, gastroenterology, pulmonology, muscular dystrophy, allergy, solid organ transplant, weight management, and adoption medicine. Many of our clinics are multi-disciplinary, which means we offer multiple specialty services within one clinic appointment. We perform outpatient procedures, pulmonary function testing, lab draws, allergy testing, and other interventions as directed by physicians within the clinic. We offer child-family-life services, which help children to cope and understand the procedures being performed in a child-friendly way. Our teams practice patient and family centered care, partnering with families to ensure they are a partner in the care our patients receive. This RN Care Coordinator will be supporting our Gastroenterology pediatric patients. ** What you will do as an RN Care Coordinator : Coordinates care across the health care continuum within designated sub-specialty population(s) Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits Facilitates proactive disease management and preventative health care using evidence-based practice Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed. Collaborates with resident and family, other inter-disciplinary colleagues, including providers, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Cares for residents in all phases of preventative care, health maintenance, diagnosis, and treatment. Responsible for the coordination of care, resident assessment, resident education and various other nursing interventions. May serve as charge nurse and delegate appropriate tasks to license and unlicensed assistive personnel that are within the job descriptions, skills, resident care needs, and the competence level of the delegate. Provides adequate assistance and support to delegates to ensure safe, reliable administration of resident care and resolution of care and service concerns. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. Qualifications Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Our interdisciplinary team of dedicated professionals cares for a wide-range of pediatric trauma and injuries, such as chest, bone, spinal cord, head and neck and abdominal injuries. We care for over 48,000 patients annually and treat almost 40 percent of all pediatric trauma in the Twin Cities. Our 28-bed Level I trauma center serves our neighbors in Minneapolis, as well as outlying facilities that depend on our critical care expertise. We offer a variety of services to our community partners, including critical care transport and telehealth. Although our specialty is pediatric emergency medicine, we are a community-based hospital that serves as a safety-net for families without a primary care provider. An informational meeting with the Patient Care Manager is required prior to awarding the position. * Based upon the unit in which you are applying, additional certification(s) may be required. Department specific required qualifications need to be obtained within 6 months of employment. * Current Pediatric Advanced Life Support (PALS) certification required or obtained during orientation. * Current Trauma Nursing Core Course (TNCC) required or obtained during orientation.
