Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Responsible for evaluating effectiveness of care plan. Essential Functions and Responsibilities : Team leads as assigned, responsible for efficient time management of peers (and self), ensuring efficient, cost-effective use of dept. resources, and ordering of supplies (may be delegated). Possess ability to perform various assessment techniques, including cardiac rhythms and airway management, to identify potential/actual problems related to anesthesia and relevant surgical procedure. Consult anesthesiologist/physician when appropriate using assertive communication skills. Utilize and troubleshoot specialty equipment (blood glucose monitor, IV pumps, Bladder scanner, ARJO lift/slides, EKG machine) in an independent, safe manner. Required: Graduate of an accredited school of nursing and holding a current license to practice in the state of Michigan.
