Home health & hospice nursing practice considers the needs of the patient, their family and/or the patient’s caregivers. The HHHRN provides care to patients across the lifespan, from prenatal to post-death periods. Home health nursing takes a holistic approach to personal health practices when treating diseases or disabilities. The practice embraces primary, secondary and tertiary prevention; assisting patients and families to coordinate community resources and health insurance benefits as well as hands on delivery of nursing services in the patient’s home. $5,000.00 Sign-on Bonus I. Major Responsibilities: 1. Assures case coordination and provides direct nursing care to patients and families on a 7 day per week (Sunday to Saturday) basis including day, evening, night, weekend, and holiday duty. 2. Meets patient care productivity standards established by the agency. 3. Collects comprehensive data pertinent to the patient’s health or the situation. 4. Analyzes assessment data to determine the diagnoses or issues. 5. Identifies expected outcomes in a plan individualized to the patient, family and/or caregivers and the situation. 6. Develops a plan that prescribes strategies and alternatives to attain expected outcomes. 7. Implements the individualized patient plan of care. 8. Coordinates care delivery. 9. Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice. I. Major Responsibilities: 1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population. 2. Position Qualifications: License/Certification/Education: Required: 1. Current Massachusetts Nursing licensure and registration. 2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies. Preferred : 1. Bachelor of Science degree in Nursing highly preferred. Experience/Skills: Required: 1. Thorough knowledge of the discipline of Nursing. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents. III. About Crotched Mountain School Nestled in the beautiful Monadnock region of New Hampshire, CMS offers a therapeutic and adventure-based learning environment for neurodiverse learners. Responsibilities Medical Coordination & Advocacy Assess and coordinate healthcare for students and residents. Maintain and review medical services and records to ensure compliance with regulatory standards. Communicate health updates with Residential Managers, the Health Care Director, and other key staff.
