We're seeking a compassionate, experienced Registered Nurse (RN) – Care Coordinator to lead with purpose and deliver quality care that changes lives.

Responsibilities

Medical Coordination & Advocacy

  • Assess and coordinate healthcare for students and residents.

  • Maintain and review medical services and records to ensure compliance with regulatory standards.

  • Communicate health updates with Residential Managers, the Health Care Director, and other key staff.

  • Work closely with physicians to obtain and implement treatment plans.

  • Attend medical appointments with individuals as needed and assist with transportation.

  • Comfortable with blood draws

Leadership & Supervision

  • Supervise and support assigned nursing staff.

  • Serve as Med Tech trainer and oversee medication administration protocols.

  • Deliver in-service training to residential staff on relevant medical topics.

  • Report staff and medical concerns to the Director of Nursing (DON).

Participant Support

  • Foster personal growth, dignity, and respect in all interactions with students.

  • Ensure all care is age-appropriate, culturally sensitive, and rights-based.

  • Identify and address any medical concerns that may affect placement or progress.

Administrative Duties

  • Complete required documentation and medical reports accurately and timely.

  • Implement assigned procedures or interventions as directed by the DON.

  • Uphold confidentiality, professionalism, and organizational values in all duties.

  • Participate in interdisciplinary team meetings and collaborate across departments.

Qualifications

  • Associate’s degree in Nursing (required)

  • Current NH RN license

  • 1+ years supervisory experience 

  • Experience with children/adults with disabilities

  • Valid NH driver’s license
