Quality Improvement: Completing discharge phone calls for readmission tracking and Quality Assurance and Performance Improvement (QAPI) purposes. Collaboration: Working collaboratively with the interdisciplinary team; will serve as a social worker representative for Rehab residents, complete PASARRs, and attend care conferences and Medicare meetings. Training: Training other nurses as directed by the Director of Nursing (DON). Qualifications: License: Current Registered Nurse (RN) license. Experience: Minimum 3 years of nursing experience, preferably in long-term care or hospital settings. Knowledge: Strong knowledge of care planning, complex discharge protocols, and regulatory requirements. Skills: Excellent communication, organizational, and interpersonal skills. The Registered Nurse-Home Health, in conjunction with the Case Manager, utilizes physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. The Registered Nurse-Home Health is responsible for planning, coordinating and documenting care for an assigned caseload of patients. Participates in Quality Assessment and Performance Improvement (QAPI) activities.
