What you'll do Performs comprehensive, age-appropriate patient assessments, incorporating medical history, social and cultural factors, and evaluates education needs for patients and families. Delivers skilled nursing interventions based on individualized care plans, prioritizing patient needs and proactively preventing complications. Develops and adjusts care plans in collaboration with patients, families, and the healthcare team, accounting for rehabilitation potential and discharge planning. Accurately documents patient care including assessments, interventions, education, and outcomes to ensure continuity and quality of care. Communicates effectively using structured methods like ensuring clear, timely and complete information sharing among team members. Demonstrates leadership and collaboration, participating in shared decision-making, mentoring staff, and taking on the charge nurse role as needed. Maintains a safe care environment, following infection control and safety protocols while managing supplies and supporting resource efficiency. Required qualifications Possess a Master’s Degree or higher in Nursing, or in a field deemed to be equivalent by the Oregon Board of Nursing. Have satisfactorily completed a Nurse Practitioner Program recognized by the Oregon Board of Nursing. Hold a current, unencumbered Registered Nurse license in the State of Oregon. Hold valid, unencumbered Nurse Practitioner national certification issued American Nurses Credentialing Center (ANCC) or National Certification Corporation (NCC), or an equivalent Medicare/Medicaid approved certifying body or be actively seeking certification and obtain the same in the specialty area requested within 24 months of privileges being granted at Salem Hospital. Hold DEA certification. At least 12 months as an Advanced Wound Care Nurse Practitioner during the past three (3) years, required. Additional information Certified Wound Care Nurse Advanced Cardio Life Support - within 6 months of hire If the Nurse Practitioner does not have the relevant experience and cannot otherwise document current competency, a monitoring plan may be required. Assess, diagnose, and manage chronic conditions for patient population. Prescribes antibiotic therapy in accordance with applicable federal, state, and local requirements. Communicates with Primary Care Physician (PCP), referral sources, home health agencies or office staff regarding changes in patient’s condition. Assess patient needs and qualifying criteria for Hyperbaric treatment. Maintains an accurate accounting of visits in the patients’ medical record. Completes patient medical record documentation within 24 hours. Contribute to the development of new services, programs, or processes. Administer medications, order diagnostic labs /tests, and perform basic procedures according to the training and needs of the patient and department. Promotes the Patient Safety Standards as a core value of the organization. Displays proper etiquette and mannerisms that reflect the Keys to Excellence. Secondary Functions: Participates in departmental and hospitalwide informational meetings and inservices, including staff meetings, hospitalwide forums, and seminars. Reviews department and applicable hospital wide policies and procedures annually. Functions as emergency hyperbaric chamber operator, as needed. All other duties as assigned.
