Summary: Reporting and accountable to the Nurse Manager, Director, the Outpatient Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in an outpatient setting. Provides competent clinical nursing care consistent with professional standards. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Responsibilities: Assesses patients either attending the outpatient clinic or via phone interview, to prepare the patient for anesthesia and the surgical process. This may include conducting preoperative assessments such as medical and surgical history, current medications, allergies, lab result review/ordering, and overall anesthesia risk evaluation prior to surgery. Follows the Peri-Anesthesia Testing Guidelines and surgeon orders. In person clinic patients will have complete physical assessments, including VS and weight. Develops and implements patient care plans, consults, or review of medical clearance in coordination with surgeons, anesthesia, and other members of the healthcare team to ensure the safety of the patient through the perioperative experience. Educates patients and their families on the procedure prep, NPO status prior to surgery, medications to take/hold prior to surgery, arrival times, postop support needs, etc. Provide information related to emotional support and counselling to patients and their families as indicated . Collaborates with other healthcare professionals involved in the patient's care, including cardiologist, pulmonologist, and primary care physicians . Advocate for and educate patients on preventative care measures, such as regular screenings, balanced nutrition, and exercise. Maintains accurate and detailed records of patients' medical histories, medications, treatments, and progress through the preoperative process. Ensure that all patient records are kept confidential and are up to date. Adheres to all hospital and departmental safety policies and protocols and ensure compliance with healthcare laws and regulations. Stay updated on the latest advancements in nursing care through continuous learning and professional development. Attend seminars, workshops, and other training sessions as required . Advocates for patients' rights and needs, ensuring they receive the best possible care. Address any patient complaints or concerns, escalating them to management when necessary. Administers patient care under the direction of a physician. Identifies and pursues expected patient outcomes in collaboration with the interdisciplinary care team. Provides competent clinical nursing care consistent with healthcare organization and nursing standards, policies and procedures. Organizes and prioritizes care according to patient/family needs. Consults with appropriate resources in a timely fashion regarding patients with complex care issues, unusual teaching needs and/or those at high risk for discharge planning. Collaborates with peers in a clinical operations environment, including fostering intra/interdepartmental communication. Synthesizes data from complex situations to determine priorities for care, including appropriate physical, psychosocial, education and safety needs. Assesses and anticipates discharge needs of individual patients and families. Communicates with patient/family to prepare or update the plan of care using cultural competency and active listening. Develops relationships with patients and their families that promote their ability to advocate for patient and family needs. Develops clinical and operational excellence in self and in peers through education and training. Promotes a safe, clean and secure hospital environment for all, including the application of standard precautions and principles of infection control. Other information: REQUIRED Must be licensed to practice as a Registered Nurse in the State of North Carolina or one of the compact states by the first day of clinical care. Basic Life Support (BLS) certification. Care Manager Registered Nurse (RN)

Summary: The Care Manager - RN provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of care management, utilization review and discharge planning. The Care Manager must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies. Responsibilities: Reviews new admissions and prioritize patients appropriately. Conducts and documents assessment and a plan of care in Electronic Medical Recordkeeping (EMR) system. Discusses with appropriate members of the multidisciplinary team when there are barriers to discharge and psychosocial concerns impacting progression of care or readmission risk. Coordinates family meetings, as necessary, to support the progression of care. Provides education on community resources, support/educational groups, and any other appropriate resources to patient , family, and care team . Educates and/or coordinates referrals to community resources and post-acute providers as necessary. Coordinates with assigned units to provide and receive information on patients’ progression. Alerts care team to concerns that could impact anticipated discharge of the patient and any care that will assist with discharge readiness. Modifies discharge plan based on information shared at the meeting. Assist with identification of the expected discharge date (EDD). Collaborates with interdisciplinary care team to problem-solve issues with complex patients and identify trends. Formulates potential solutions with Utilization Manager and Social Worker and continuously monitor cases/follow up on all action items. Proactively identify high risk cases that need to be escalated to the list that are not scheduled for discussion that week. Complete CCM follow-up after the meeting as assigned. Maintains a system for monitoring admissions and assures review of continued stays in a timely manner . Monitors all observation patients throughout the day to ensure appropriate progression of care. Interfaces with other hospital departments in matters of review decisions, coding, discharge planning, and UR issues. Serves as a liaison with insurance companies and government agencies to coordinate information needed for approval of hospital procedures/services rendered . Maintains proper documentation for third party payers and other agency requirements. Performs admission and continued stay reviews to determine the appropriateness of hospital admission based on approved criteria and standards per the Hospital Utilization Review Plan. Directs delivery of notifications to patients to deliver notifications.
