Conduct pre-procedural evaluations and obtain informed consent for HV patients. Manage post-procedural care, including wound and device assessments, patient education, and medication reconciliation. Perform rounds on existing and new consults, as time and patient volume permit. Participate in designated weekend rounding responsibilities in collaboration with on-call cardiologists. Provide comprehensive transitional care services within the cardiology clinic to ensure continuity of care post-discharge. JOB ACCOUNTABILITIES: ADVANCED NURSE PRACTICE: Manages the medical care for patients utilizing clinical protocols in collaboration with the physicians. Performs a physical exam if indicated. Ensures individualized health maintenance status to include, but not limited to, vaccinations, screening for risk factors, mental status baseline, dental screening and other appropriate functional assessments. Conducts diagnostic examinations, complies and updates detailed medical histories. Consults with supervising physician in cases of abnormal examination of test findings and when implementing clinical protocols. Critically ill children are cared for with such treatment modalities as mechanical ventilation, inhaled nitric oxide, ventricular assist devices, temporary pacemaker therapy, peritoneal dialysis, continuous renal replacement therapy and acute hemodialysis. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents.
