Department: 04170 WFBMG University Group Practice: WFBMC Main - Hospitalists Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Summary: The FLEX position was created to provide additional coverage across the system to help with staffing shortages, PTO coverage, admin time, sick leave, and demands of high census. Locations: HPMC, LMC, DMC, WMC, and AHWFBMC Schedule: 7 on, 7 off APP will follow early/late protocol at each specific site Supervising physician: must be privileged and payor enrolled at all WFB sites. Reporting: The Flex APP will report to the APP leader responsible for flex staffing. Determining location/schedule The FLEX APP schedule will be made every 3 months with collaboration between Melissa and APP leader to help cover open shifts across all sites. If there are no open shifts, the APP will be scheduled to work at the site that most consistently demonstrates additional needs. All efforts will be made to minimize the number of different services or locations each week. Attention will be given to try and ensure at least 2 days in a row on a particular rounding service and less than 2 different locations in a given week. FLEX’s schedule will be subject to change for callouts, emergencies, or high census. Any change will be communicated to the FLEX APP no later than 6pm the day prior. Swing shifts : FLEX APPs will be assigned to swing shift not to exceed 6 weeks per year. If scheduled to work a swing shift, APP will not be scheduled for day shift the following day. Wilkes coverage will receive a $100/shift incentive for coverage. Compensation The FLEX APP will get a $10,000 annual stipend in monthly payments in addition to baseline salary. If the APP is not full FTE FLEX, they would receive a portion of the stipend consistent with the percent FTE they work as FLEX. FLEX position is created as a 1.0 FTE position and anything less than this will require approval by APP leader. Any additional compensation given for traveling would not apply to the FLEX APP. Bonus If APPs across the system are eligible for bonuses, the FLEX APP would also have opportunity to get a bonus. Bonus would be determined by the APP manager. Bonuses will be based off a metric system, which incorporates professionalism and cooperation with the FLEX schedule, keeping up with communications/OPPEs/department responsibilities, and then involvement in other initiatives that can be done from a network standpoint such as serving on a committee, publishing papers, and participating in team events. Pay Range $53.80 - $80.70 EDUCATION/EXPERIENCE: Graduation from an accredited Physician Assistant Program, Master's degree preferred LICENSURE, CERTIFICATION, and/or REGISTRATION: Current applicable state Medical Board Medical License Certified by NCCPA ESSENTIAL FUNCTIONS: Using medical decision-making skills to determine appropriate patient care plan. Job Description: The Pod Provider delivers comprehensive chronic disease management (e.g. hypertension, diabetes, chronic kidney disease), evaluation of acute concerns, behavioral health condition management (i.e. depression, anxiety, alcohol and substance use disorder), and preventative care and wellness support (including sexual and reproductive health). The role works collaboratively with an interdisciplinary team of clinical and non-clinical members to develop member care plans and to support members throughout their care journey. Proactively outreach to members who need follow up care. Complete comprehensive and compliant diagnosis capture related to HCC (Hierarchical Condition Categories) and ICD-10 (International Classification of Diseases-10) Coding. Address contractual and company-prioritized quality gaps and ensure proper chart documentation and codes (ICD or CPT) are included in the encounter as supporting evidence of gap closure. Prescribe 1st and 2nd line medications for depression, anxiety, alcohol use disorders and opioid use disorders. Remain up to date on evidence-based care for complex patients in primary care: (adult only, pediatric in certain markets). Refer member to specialist care appropriately and according to internal CBH guidance if available. Support the member in achieving the goals in their care plans (physical, behavioral, and/or social health). Participate in case conferences with the care team, providing clinical expertise on care plan development. Provide guidance on highest impact clinical interventions and tasks to achieve a member’s goals. Collaborate on a panel of members to provide clinical input, including health maintenance, chronic disease management and co-occurring psychiatric disorder support. Job Summary Novant Health Transplantation and Cellular Therapy Program is seeking an allogeneic transplanter. We are a FACT accredited Transplantation and Cellular Therapy Program, performing autologous transplantation, CART cell therapy, and allogeneic transplant. Our program currently has eight physician providers and 15+ advanced practice providers (APPs) divided between the Charlotte and Winston-Salem sites. Although there is a high level of cross site collaboration, both sites operate independently for patient care including rounding and call coverage and providers cover just one site. The planned rounding and call schedule is 1:4 with the APPs taking 1st call. There is excellent support from the pathology department with two certified hematopathologists, blood bank, radiology, intensive care and other specialist services. With the addition of Dr. David Rizzieri, formerly of Duke University, to lead the Novant Health Cancer Institute, the successful candidate will join a robust and growing team of clinical researchers in Cellular and Immunotherapies.
