Job duties can include, but are not limited to: Completing an initial assessment of patient and family to determine hospice care needs. Regularly re-evaluates the patients nursing needs. Use assessment data to determine care plan needs. Collaborate with the healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated. Counsels the patient and family in meeting end of life needs Initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's On-Call rotation every 6 weeks The proper candidate must have a current Ohio RN License in good standing, must be familiar with and willing to travel the area and have hospice experience. This is a fantastic full time position allowing the chosen candidate to work one week on followed by one full week off while still maintaining full time hours, benefits and pay. They will respond to manage distress calls,do new admissions, provide nursing care, emotional support and education to terminally ill patients and their families. POSITION SUMMARY: We are looking for compassionate and dedicated Part-Time LPN for for our Youngstown hospice agency. LPN's maximize the comfort of patients and families in accordance with the interdisciplinary plan of care. POSITION REQUIREMENTS: Graduate of an accredited practical nurse or vocational nursing program. Currently licensed as an LPN/LVN in the practicing state. Three years nursing experience. Community health/hospice or medical/surgical experience is preferred. Demonstrates good verbal and written communication, and organization skills. Valid driver’s license with an automobile that is insured in accordance with state requirements. Understands the hospice and the needs of the terminally ill.
