Serve as liaison to physician: Serves as the "point person" who communicates and assists with understanding of dashboard metrics and opportunities to improve patient care, internal processes and work flows to the physician and also rounds with physicians to discuss the patient's clinical status and ensure that critical information is shared. Facilitate quality improvement, including identification and dissemination of best practices: Reviews data and trends from reports generated by Summit Health to determine system performance related to patient outcomes and then suggest areas for improvement. For example, review adherence with ACO, NCQA and contractual reporting requirements compared with the EMR. Identifies education, workflow, process or documentation/template enhancements. The coordinator meets with RN Clinical Manager, and/or Director of Clinical Operations to discuss individual nurse performance issues and/or determine whether a system-wide performance improvement initiative is warranted. Mentor and coach support staff: Clinical coordinator orients all new clinical staff to ACO, PCMH requirements, provides ongoing mentoring, coaching, and education to all clinical support staff members.
