The Montefiore Licensed Practice Nurse contributes standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement. The Licensed Practical Nurse actualizes the vision, mission, values, balanced score card performance measures for Montefiore Medical Center.

The LPN participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The LPN demonstrates core and unit specific competencies based on the patient population served.

The Licensed Practical Nurse contributes to the actualization of the vision, mission, values and philosophy and balanced score card performance measures for the Medical Center. He/she is committed to maintaining the standards of the practice of nursing and is responsible for the delivery of care to the neonate, pediatric, adolescent, adult and/or elderly patient within their scope of practice. He/she participates in performance improvement, research activities and maintains clinical expertise

Responsibilities

  • Provide patient/family teaching and discharge instructions to meet learning/ discharge needs as identified by RN.
    • Initiates measures for patient support.
    • Identifies and reports needs in 100% of assigned patients.
    • Implements content and teaching methodologies for cognitive, functional, maturation levels identified by the RN.
    • Facilitates discharge in collaboration with RN.
  • Able to set priorities, problem-solve, and use proper judgment in difficult situations and be flexible in non-emergent situations.
    • Takes action within scope of practice to optimize achievement of patient outcomes.
    • Differentiates between priority and non-priority work.
    • Complete assignment within established time frames.
    • Escalate patient issues following established communication systems.
    • Participates in identification, investigation and resolution of actual/potential risk issues.
    • Adapt to change in the work environment, manage competing demands, change approach or method to best fit the situation.
  • Provides and maintains safe environment for patients/families through preventive and interventional measures.
    • Administers medications with 100% compliance within scope of practice and MMC policies and procedures.
    • 100% compliant with institutional P&P/Regulatory Agency Guidelines.
    • Adheres to MMC's infection control, safety & disaster policies and procedures.
    • Orients 100% assigned patients/families to environment ensuring comprehension of information presented.
    • Participates and responds to investigation and resolution of actual and/or potential risk issues.
    • Uses equipment in accordance with MMC policies and procedures.
  • Documents direct patient care as per MMC policies and procedures.
    • Records medication administration, specimen collection and other assigned tasks as per MMC policies and procedures with 100% accuracy.
    • Records nursing interventions and observation of patient response according to MMC policies and scope of practice accurately and timely.
    • 100% completion of assigned patient admission/advance directives/discharge documentation as per MMC policies and procedures.
  • Participates, practices and promotes continuous quality improvement.
    • Contributes to problem identification and problem solving in the work environment.
    • Recognizes responsibility extends beyond one's own patient population; supports and assists co-workers.
    • Participates in collection and documentation of data relevant to planning/evaluation of patient care.
    • Participates on work groups or data collection annually.
    • Demonstrates accuracy and thoroughness in work.
  • Communicates effectively with patients/families and members of the health care team.
    • Incorporates principles of family centered care into daily practice.
    • Communicates variances/observation of patient status to RN accurately and timely.
    • Ensures patients' rights are respected as identified in the Patient Bill of Rights at all times.
    • Demonstrates respect of cultures/cultural diversity of patients/families and health care team members.
    • Recognizes the family as defined by the patient.
    • Describes and adheres to established therapeutic relationships with patients/families.
    • 100% legible documentation.
    • Completes 100% of required documentation for assigned unit patients.
    • Collaborates with registered nurse and co-workers to resolve patient care problems.
    • Utilizes communication system as defined by table of organization and escalation policy to problem solve.
    • Reports on and off duty to designated RN 100% of the time.
    • Ensures call bells are answered in a timely manner 100% of the time.
    • Speaks clearly and persuasively in positive or negative situation.
    • Listens and seeks clarification.

Requirements

  • Responsible and accountable for own practice.
    • 100% compliance with MMC departmental policies and procedures.
    • Adheres to MMC Human Resources Policies and Procedures including attendance, punctuality and dress code.
    • Supports the mission, vision, critical success factors and goals of MMC and standards of nursing.
    • Completes all mandatory LPN nursing competencies including BLS, IV certification, glucose monitoring.
    • Attends educational programs as scheduled.
    • Accepts direction/change in assignment as deemed necessary by RN.
    • Includes appropriate staff in decision making process.
  • Other duties as assigned.
    • Completes duties as assigned.
  • Sterilize instruments and equipment using the autoclave.
    • Accuracy in sterilization and documentation as per MMC policy.
  • Order and maintain medications and supplies to par level for site/s. Ensure exam rooms are appropriately, cleaned and stocked each session between patients.
    • Practice is properly stocked and exam rooms remain clean and stocked with supplies every session and between patients as required.
  • Collect and properly label specimens as per MMC policy. Maintain specimen (blood, biopsy) logbooks and report abnormal results to MD.
    • Accurate labeling and documentation of specimens required. Zero tolerance for error.
  • Great and direct patients to exam room.
    • Complete vitals and all other clinical duties directed from provider or RN. Patients triaged and roomed within 10 to 15 minutes of arrival to practice.


Qualifications
  • High School.
  • 1-3 years.
  • Effective English communication skills, written, oral and interpersonal.
  • Ability to use basic data entry and retrieval system.
  • Additional education required by specialty areas/patience care locations.
  • OHS Annual Assessment.
  • Licensed Practical Nurse New York.
  • OHS Fit Test.
  • NYS License for Practical Nurse and current registration to practice as an LPN in New York State.
  • Basic Life Support License.
