Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns. What is Required? Graduate of an accredited school of professional nursing Current Certified Licensed Vocational Nurse or Licensed Practical Nurse 1 year of home care or hospice experience Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations Able and willing to travel up to 50% as needed to support business and patient needs.
