JOB DESCRIPTION: SUMMARY OF POSITION Functions as staff LPN utilizing the nursing process with performing comprehensive psychiatric /mental health assessments. Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the state specific Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician. ESSENTIAL DUTIES & RESPONSIBILITIES Performs comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary. Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states. Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS. Provides phone triage and addresses medication issues with consumer when necessary. Provides proper education and guidance as necessary. Assists patients with accessing medications including obtaining prior authorizations and accessing prescription assistance programs. Provides healthcare education to clients and/or families or staff as assigned. Provides CLIA Waived laboratory testing as ordered. Maintains CLIA waived laboratory environment including Quality Control testing and documentation. Monitors received laboratory values and reports abnormal values as required. Phlebotomy required. Provides employee health services as directed, including annual influenza vaccination clinics and staff TB skin testing. Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary. Nurses may provide direct physical supervision to Certified Medical Assistants and delegate tasks in accordance with the Tennessee Nurse Practice Act and other applicable regulations. Completes clinical documentation within organization’s policy standards. Maintains all mandatory trainings up to date. May provide assigned residential nursing services as directed. KNOWLEDGE, SKILLS & ABILITIES Must have the ability to work cooperatively with all staff and physicians and function as a valuable member of the treatment team. How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Additional Details 1430-2300 - 5 shifts a week and EOW rotation Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
