Maintains a systematic approach to pertinent data collection from the patient or family/caregiver as appropriate and documents data in the electronic medical record. Collaborates with patients, providers, family, significant others and other healthcare professionals to obtain pertinent data to help develop plan of care. Collaborate and convey all data to provider(s). Planning: Develops an individual plan of care and goals with patient, family, significant other and providers. Identifies and prioritizes expected outcomes and goals in planning patient care based on wound healing/hyperbaric medicine best practices and program guidelines. Identifies and involves the multidisciplinary team to meet each individual patient needs and concerns through the development of goals and anticipated outcomes. Maintains communication between interdisciplinary team to carry out plan of care (e.g. Hyperbaric medicine, visiting nurses, primary physicians and other healthcare professionals as needed). Responsibilities Registered nurses (RNs) perform basic duties that include treating patients, educating patients about mental and medical conditions, and providing advice and emotional support to patients� and/or family members, running therapeutic groups, psychiatric crisis intervention, seclusion and restraint of patients. RNs record patients� medical histories and symptoms, help to perform diagnostic tests and analyze results, operate medical machinery, administer treatment and medications, and help with patient follow-up and rehabilitation. RNs teach patients and their families how to manage their illness or injury, diet and exercise programs, and self-administration of medication and physical therapy. Other related duties. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Must have current and valid State license as a Registered Nurse.2. Current BLS certificate upon hire and maintain current.3. Working knowledge of Mental Health Laws and practice standards. 4. Working knowledge of interdisciplinary treatment team process, behavioral health programming and daily nursing functions for inpatient and outpatient behavioral health units. 5.
