Job Summary The Providence Center Child and Family Services Healthy Transitions Registered Nurse (RN) will provide nursing services for ages 16 -25 with mental health and/or co-occurring disorders, which serves clients and their families in the community. Will assess, plan, develop, coordinate, and provide treatment and supportive services to children with mental health issues and their families. Services will be provided in the community. Coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence through providing outpatient psychotherapy either at The Center or in the community. The Healthy Transitions Nurse must have professional knowledge and understanding of the characteristics of children and young adults with mental health issues, their families, and family systems. Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills. Ability to work well with community partners and in a team environment is essential. Highly motivated, self-directed, willing to work a variety of hours, including evening and on-call. Schedule: Monday through Friday 8:30 am – 4:30 pm with an half an hour for lunch. This position also requires some evening hours. Duties And Responsibilities Perform psychiatric nursing in accordance with appropriate medical protocol. Provide accurate and timely information about medications and their side effects to the client and relevant others. Use medication maintenance strategies, determining that the doctor’s orders have been accurately transcribed, implementing appropriately the prescribed medical regimen, monitoring lab results to validate medication compliance and assessing any aspect of the consumer’s treatment regimen that endangers the level of wellness, communicating questionable aspects of consumer care to appropriate persons. Use physical assessment skills to determine related nursing interventions, including when medication exacerbates medical conditions and recognition of basic medical problems. Inform and educate clients about mental illness, medication, coping skills, their rights as clients, services, community resources, self-help and the consumer movement. Utilize best practices of intervention and support strategies. Teaches both simple and complex skills, including physical, social, cognitive emotional, and other relevant skills. Incorporate consumer choice in treatment. Make period reassessments and if warranted, change treatment plans. Demonstrate appropriate use of range of crisis prevention and intervention strategies, including crisis prevention and intervention, risk assessment, hospital alternative, hospitalization, and assessment of self care, performing calmly and professionally during crisis or emergency situations. Coordinate activities regarding consumer hospitalization, serving as liaison with the hospital regarding admission, case review, and discharge. Comply with written clinical procedures and professional standards. Address the unique needs of individuals and co-occurring disorders (e.g., substance abuse, developmental and/or physical disabilities, personality disorders, trauma, brain injury, etc.) that consumers have. Keep current about new developments in psychotropic medication and other biological interventions. Make assessments and provide appropriate crisis interventions and risk management as needed. Teach consumers adult daily living skills. Initiate treatment and advocate for consumer within network of care. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves. Transport consumers in own vehicle or TPC van to planned activities and appointments. Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by Center confidentiality policy. Maintain cooperative relationships with other employees, consumers and the public. Attend trainings and conferences. Serve on appropriate Center committees. Requirements Associate’s degree in Nursing required; Bachelor's degree from accredited four-year College or University preferred. Rhode Island Registered Nurse (RN) license or Compact License required. Experience In Human Services Or Outpatient Setting Preferred. Experience working with psychiatric and substance use disorder patients also a plus. Bilingual Spanish preferred. Must have valid driver's license and insured auto. Must be able to work flexible hours. Job Summary The Providence Center ACT Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications. Coordinate activities regarding consumer hospitalization. Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay. Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including MD. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences as needed. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees. Partake in training new hires and student Nurses. May be asked to share on-call with other nursing and program staff.
