Licenses/Certification: Current Massachusetts state RN license All RNs must have current BLS certification ACLS is required with BLS for RN positions on the following units/departments: Telemetry, ICU, ED, Cardiac Rehab, Endoscopy, LDRP, PACU, Med/Surg NRP is required with ACLS and BLS for RNs on LDRP PALS is required with ACLS and BLS for RNs on PACU and ED Required Qualifications and Skills: New Graduate Nurses accepted for some positions. Delegate care in accordance with the Massachusetts Nurse Practice Act and Sturdy Health guidelines. Maintain accurate and detailed patient records, including assessments, interventions, and responses to treatment as well as individualized nursing plans of care. Provide emotional support and guidance to patients and their families, helping them cope with the challenges of illness and treatment. Educate patients and their families about their conditions, treatment options, and strategies for maintaining health and preventing illness. Respond to medical emergencies, providing first aid and initiating appropriate interventions. Ensure patient confidentiality is maintained at all times. Utilize and maintain medical equipment (e.g., monitors, IV pumps, ventilators). Participate in quality assessment and improvement initiatives and contribute to the development of best practices in patient care. Develop and maintain positive relationships across all areas of responsibility. Visibly demonstrate commitment to customer relations and all basic concepts of continuous quality improvement and preventing error from reaching the patient. Charge Nurse duties as assigned. Job Summary The Providence Center Unity Team is looking for a motivated, team-oriented Registered Nurse , to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per doctor orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications. Coordinate activities regarding consumer hospitalization. Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay. Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including MD. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees. Partake in training new hires and student Nurses. May be asked to share on-call with other nursing and program staff. Other duties as assigned. Requirements Associate’s degree (ASN) in Nursing required; Bachelor's degree (BSN) from accredited four-year College or University preferred. Rhode Island Registered Nurse (RN) license required. Experience In Human Services Or Outpatient Setting Preferred. Experience working with psychiatric and substance use disorder patients also a plus. Bilingual Spanish preferred.
