Assesses patient utilizing rehabilitation nursing policies and procedures which describe rehabilitation nursing standards and scope of services, in addition to rehabilitation specific patient care. Performs duties demonstrating a rehabilitation knowledge base. Using hospital and regulatory standards of care for rehabilitation, the nurse demonstrates sound clinical judgment in the development, implementation and evaluation of the nursing aspects of the trans-disciplinary care plan. In conjunction with the patient/family, the nurse develops an individualized patient care plan and determines interventions based on assessment/reassessment findings. The nurse evaluates patient outcomes on an ongoing basis, establishes priorities for patient care services as evidenced by modification of care plan, if necessary, and demonstrates the ability to accurately document patient care activities in the medical record. Utilizing a case management approach which initiates discharge planning on admission, the nurse plans, and guides, directs, coordinates and collaboratively incorporates the overall aspects of patient care across the continuum by effectively communicating with physicians, staff and other disciplines. Demonstrates respect for patient rights. Answers telephone and patient call lights in a prompt and courteous manner, addressing callers and patients appropriately by name and/or title. Performs efficiently in crisis and emergency situations, following established protocols, remaining calls, informing appropriate persons and documenting events. Demonstrates flexibility with changing patient assignments, varying staffing levels and patient care environment. Completes work on time. Supports and adheres to established departmental policies and procedures, objectives, safety and infection control standards, including compliance with punctuality, overtime, floating, staffing/scheduling, body mechanics, TB screening, JC update, attendance, English language, ID badge, and dress code standards.
