Reporting to the service line director, you will be responsible for coordinating and leading the planning and implementation of clinical initiatives to improve the quality, efficiency, and cost-effectiveness of care and will be instrumental in creating workflow process for e lectrophysiology procedures, atrial fibrillation referrals and and regional outreach to providers for EP referrals. Collaborate closely with a multidisciplinary team and identified physician leaders to optimize patient outcomes and promote evidence-based practices. Work in tandem with the heart failure and structural heart teams to enhance care coordination and ensure that appropriate patients receive timely procedures. Strive to provide timely access to treatments, reduce readmissions, improve clinical outcomes, enhance patient satisfaction, and effectively manage financial resources. Accountable for the effective management and coordination of care of patients primarily being evaluated for cardiovascular procedures. Partner with the interventional cardiologists, cardiothoracic surgical team, and clinical staff of multiple departments, both inpatient and outpatient within Baystate Health and external local and regional healthcare facilities to coordinate the care of cardiovascular patients who are being evaluated and selected for cardiovascular procedures through their pre- and post-procedure care. Works in collaboration with a multidisciplinary team to plan, direct, and evaluate the care provided to patients and their families. This coordinator is a front-line nurse leader with excellent leadership, communication, and interpersonal skills. The coordinator possesses expert clinical knowledge, which is applied to systematically and continuously collect and assess data in collaboration with the multidisciplinary team and the ACC-NCDR Data Coordinator to provide and improve quality patient outcomes for the patient and Baystate Medical Center's Heart & Vascular program. Collaborates with inter-professional team members to enhance outcomes across the continuum of care by utilizing, critiquing, and/or conducting research to promote evidenced based practice. Evaluate current clinical practices of referring providers and staff and provide education to ensure research-based best clinical practice is delivered to patients. The Heart & Vascular RN Coordinator provides leadership as a member of the multidisciplinary team collaborating in the development, execution, and evaluation of their respective Heart and Vascular program. Communicates professionally and effectively with the faculty and staff in planning for programmatic changes using tact, discretion, and diplomacy; is skilled in problem-solving techniques, conflict resolution, and team building development, and provides feedback to team members appropriately. Assist and support patient and family in understanding and navigating through a complex clinical eval, maintain expert clinical knowledge and advanced certifications in nursing practice with a special focus in the specialty to be a strong clinical resource. It also stands for Baystate’s many opportunities for growth and learning in the region’s only academic medical center and within our community hospitals, and it stands for our commitment to taking care of all those in the community who need us DEPARTMENT: Heart and Vascular Critical Care (HVCC): The Heart and Vascular Critical Care Unit provides a full array of services for the critically ill cardiac patient to include care of the post or evolving MI, immediate and post cardiac surgery care, immediate cardiac interventional care, therapeutic hypothermia for post cardiac arrests, titratable vasoactive infusions, hemodynamic monitoring requiring the use of specialized critical care equipment, and mechanical ventilation support. The majority of patients have a diagnosis of STEMI, cardiac arrest, CHF, and post cardiac surgery with CABG, valve replacement, and TAVR’s. The team is trained to support several advanced cardiovascular and critical care modalities including Intra-aortic Balloon Pumps (IABP), Impella CP, 5.0 and RP, LVAD implants with Heartmate 3, Extra-corporeal Membrane Oxygenation (ECMO), and Continuous Renal Replacement Therapy (CRRT). Surgical Trauma Intensive Care Unit (STICU): Surgical ICU equipped with advanced hemodynamic monitoring and state-of-the art continuous renal replacement technology. The only major referral care center and the only Level 1 trauma center and pediatric trauma center in the area. RESPONSIBILITIES: Collects patient data to be used by the licensed health care provider in the comprehensive care of the patient.
