What You’ll Do Conduct full health assessments, exams, and medication reviews. Create personalized care plans with the interdisciplinary team. Educate residents and families on staying healthy and independent. Keep clear, up-to-date records in eClinicalWorks. Participate in quality initiatives that truly make care better. What You’ll Bring Master’s degree from an accredited NP program. Active New York State NP license and DEA registration. Eligible for Medicare/Medicaid billing. 3+ years of clinical experience (geriatric or long-term care experience is a big plus). A great bedside manner, a big heart, and a good sense of humor.
