The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs. Home Care Services Program (HCSP) is recruiting for three (3) Staff Nurses, to function as a Staff Nurses in HCSP/ CASA North, South, and Staten Island offices, who will: - Conduct in-home or virtual assessments at the request of HRA using a State approved assessment instrument to evaluate the client’s ability to perform activities of daily living and the degree to which the client requires assistance for each function and task; the completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of the plan of care must take into consideration client’s home environment and social support system. - Complete home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. The reviews consist of screening all documents related to the request for service and may include contacting the client’s physician or other resources for additional medical documentation and information as necessary. - Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation. - Prepare documentation for and testify at Fair Hearings and at other legal proceedings to explain clinical findings and determinations regarding the plan of care. - Provide consultation to HRA Medical Review Team and social service staff at HRA’s request. - Complete self-audits of State approved assessments and medical review of charts. - Work toward obtaining PRI/screen certification and conduct assessments of clients transitioning to a Nursing Home Facility. Oversees the direct implementation of nursing care to ensure that it is carried out in accordance with the plan of nursing intervention, and overall treatment/service plan, and to ascertain if the needs of the client require modification of the plan. Coordinates nursing care activities with other professional disciplines to ensure that the nursing plan is consistent with and integrated with the total treatment/service plan Participates at team meetings and provides nursing input on the development and implementation of treatment/service plans. Conduct in-service training with regard to personal hygiene, medication administration, family life and sex education. Supervise medication administration practicums and sign off on medication administration monthly. Control communicable diseases and infections through: identification and assessment, reporting to medical authorities, and implementation of appropriate protective and preventive measures. Record on the residents' medical records nursing observations and action taken, such as nursing care given, medications and treatments given, reactions to treatments, tests, the intake and output of fluids and solids and the resident's vital signs. Record nursing needs of residents. Modify the nursing plan, in terms of the residents' daily needs, at least annually for adults in accordance with developmental and medical changes. Be available for on-call duty and to work varied shifts as the need arises. Work with all consulting and/or contracted medical service providers to in-sure appropriateness of service & to insure that recommended treatments are appropriate and are carried out, integrated and documented into the treatment plan. Ensure that the inventory standards for medicines, solutions, supplies and equipment are maintained to meet nursing care needs. Account for the issuance of controlled drugs and narcotics and inventories of these drugs. Ensures resident protection from health hazards such as transmission of infectious organisms, isolation of infected residents and use of properly sterilized equipment when its use is necessary. Provide appropriate information to the resident and family about such areas as sustaining and preventive health care, personal hygiene, methods of family or self-treatments, self-medication and possible side effects of medications. Serve as liaison, resident's interpreter with outside health services providers and escort residents to services as needed. Train designated staff, by way of demonstrations and written and oral instructions, in the performance of select nursing techniques and the use of nursing equipment, and client specific condition trainings. Provides health education/teaching for clients and their families. Participates in risk management and quality assurance concerning health related issues. Provides emergency care and crisis intervention Accesses medications and observes and records effects and side effects. Participates in professional activities to improve personal skills and knowledge. Provides Nurse Vacation coverage, phone availability during workday hours, and a minimum of one site visit at covering site, for Nurse Responsibility review and weekly reporting. Follow up on any discrepancies immediately. Ensuring an adequate refill supply at all times by logging into refill alert and communicating with the pharmacy and medical doctor as necessary Medical Assessments Annual, Semi, Quarterly, Self-medication assessment, 24 hour Nursing Care and Medicaid Certifications as needed Consumer Weights Complete within the first 7 days of the month and follow up on any significant increases or decreases immediately Bowel Movement Ensure Consumers bowel movement are charted consistently and implementing prescribed protocol should any discrepancies be noted Weekly Medical Reports- To be disseminated at the end of each week to the team. Capturing a recap of all appointments from the week and all follow up recommendations. Hourly rate: $17.50- $26.00 Bonus eligible up to $600/month Job Requirements Education · High School Diploma required · Graduate of a Medical Assisting Program preferred Work Experience · Medical Assistant experience preferred · Front Desk experience preferred Required Licenses/Certifications · Basic Life Support (BLS) required at the time of hire, obtained through the American Red Cross (ARC) or American Heart Association (AHA). · One of the following certifications is required at the time of hire: · CCMA- Certified Clinical Medical Assistant (verified through NHA, National Healthcareer Association) · CMA- Certified Medical Assistant (verified through AAMA, American Association of Medical Assistant) · RMA- Registered Medical Assistant (verified through AMT, American Medical Technologists) · NCMA- National Certified Medical Assistant (verified through NCCT, National Center for Competency Testing) · NRCMA- Nationally Registered Medical Assistant (verified through the National Association for Health Professionals) In lieu of an approved MA Certification, we will also accept the following certifications at the time of hire: · Graduate of an Accredited School of Nursing or LPN or RN · EMT - Emergency Medical Technician (verified through the National Registry of Emergency Medical Technicians) · CNA - Certified Nursing Assistant Essential Functions · Conduct patient registration, which includes securing required paperwork and insurance documentation, and verification of insurance and patient eligibility. · Process co-pays and schedule patient appointments for assigned center. · Coordinate provider referrals to assigned Specialist. · Answer patient questions regarding their care and keep them informed of their status. · Inform technical staff of patient flow. · Prepare patients for examination, which includes conducting interviews to verify patient information, record medical history; confirm purpose of visit · Perform preliminary physical test (blood pressure, weight, temperature, etc.) for vital signs and escalate critical cases to provider as needed. · Conduct point of care testing as needed (i.e., flu, strep, urinalysis, ECG, HCG); and provide patient information to provider. · Responsible for stocking supplies and maintenance of exam rooms, front desk, and reception area; ensure providers have medical supplies needed. · Responsible for completing order sheets to maintain supply inventory levels; regularly check expiration dates on supplies. · Responsible for maintenance of medical records and scanning charts to patient files. · Answer phone calls, collect and sort daily incoming and outgoing postal correspondence · Ensure reception is well maintained · Conduct courtesy call-backs for patient follow-up. · Perform opening and closing duties, which includes cashing out, end-of-day communication, and securing the building. · Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed. · All other duties as assigned.
