These duties can be performed remotely but you must be available to take the calls when you are scheduled to work. The ideal candidate must be licensed in Michigan and complete CCM visits monthly. Typical schedule may be varied and shall include weekdays, holidays, and evenings. You should have experience in one of the following specialties: acute care, family health or geriatrics. Patience, problem-solving, and good communication skills are important. Patient Care Associate- 5 West

Assists nurses with direct and indirect patient care activities. Prepares patients rooms for occupancy after discharge. Additional responsibilities includes blood draws and EKG's. Essential Functions and Responsibilities: Observes patient's mental, emotional and physical conditions and report any changes to the nursing staff. Performs non-critical medical and therapeutic procedures based on medical instructions as directed by nursing staff. May perform special procedures as required and directed (i.e., blood draws/EKGs). Conducts regular patient rounds and meets with patient’s family members when required. Collects test specimens and performs laboratory analysis as directed by nursing staff. Answers patient calls and determines how best to help them. If the LPN is appropriately qualified, prepares and administers oral, rectal, subcutaneous, intramuscular, intradermal and topical medications to patients per policy and procedure.Observes patient response to medication, records unexpected reactions and reports them to the registered nurse. Identifies common patient problems, symptoms and reactions through monitoring Participates in the development, review and evaluation of the plan of care appropriate to the age of the patient in conjunction with the registered nurse.
