The primary purpose of this position is to provide quality nursing care to residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by your supervisor. Areas of care include bathroom assistance, general personal hygiene (bathing, dental hygiene, hair and nails) dressing, mobility assistance and food intake, and other needs in keeping with the individuals’ care requirements. Maintains resident rights and confidentiality. Documents all ADL’s in electronic health record. Observes and reports symptoms, reactions and other changes in condition promptly. Routinely turns and positions residents as required by care plan. Complies all aspects of residents’ rights. Is responsible for promptly reporting any incidents or evidence or resident abuse or violations of resident rights to the charge nurse or administration. Honor resident right to refuse care, report such refusal to nurse supervisor. Assist in maintaining a safe, neat and clean environment. Answers resident call bells promptly and courteously. Ensure that residents who are unable to call for help are checked frequently. Takes and documents temperature, pulse and respirations rates, food and fluid intake and output, and heights and weights and records in EHR. Candidate must be willing to travel. $100 a visit. Must be available within 48 hours to perform face-to-face visits. Accreditation from an approved certifying body for advanced practice nursing. Preferred certification in a specialist area such as hospice or pediatrics. Duties & Responsibilities: Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering, and interpreting results from lab and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical records. Manages health conditions and terminal illnesses by developing plans of care, prescribing medications and/or treatments, obtaining consultation or making referrals and coordinating hospice care services. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Reviews clinical information, and coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Completes, maintains, and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. Ensures compliance with the Medicare conditions of participation and other state regulations governing the provision of healthcare. Builds from the resources in the community to plan and direct services to meet the needs of individuals and families.
