Experience & Education: Master's degree with a minimum of one (1) year of experience as a nurse practitioner; hospice or palliative care experience preferred. CPR certified. DEA licensed. Accreditation from an approved certifying body for advanced practice nursing. Preferred certification in a specialist area such as hospice or pediatrics. Duties & Responsibilities: Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering, and interpreting results from lab and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical records. Manages health conditions and terminal illnesses by developing plans of care, prescribing medications and/or treatments, obtaining consultation or making referrals and coordinating hospice care services. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Reviews clinical information, and coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Completes, maintains, and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. Ensures compliance with the Medicare conditions of participation and other state regulations governing the provision of healthcare. Assesses and evaluates the patient’s status by writing and initiating a plan of care, regularly re-evaluating the patient and family/caregiver needs, and participating in revising the plan of care as necessary. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. This is an on-the-road position with weekly pay ranging from $22 to $24 an hour. 3 days a week from 8a-4:30p. The hospice aide is responsible for observing the patient, reporting these observations, and documenting observations and care performed. Duties & Responsibilities: Performing personal care needs after given a patient’s assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2) Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Assisting in dressing and undressing patients as assigned. Planning and preparing nutritious meals, including shopping, as assigned. Assisting in feeding the patient as assigned. Taking and recording oral, rectal and auxiliary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency.
