Job Description Coordinates care for Breast Cancer patients working with the physicians and multidisciplinary team. Principal Accountabilities Serves as patient and family advocate. Assists patients with scheduling of appointments to expedite the plan of care and to promote continuity of care. Assists in education of the patient/family to understand Breast disease process, care map and treatments. Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient. Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment. Assists with Breast MDCs. Identifies Barriers to Care and refers to support staff. Participates in Breast Cancer Support Group. Provides Treatment Summaries' and Survivorship Plans with the physician. Participates in Breast Programs and community activities. Performs other related duties as assigned. Required QUALIFICATIONS Bachelor's in Nursing or related field. Valid State or national Nursing License. 2 years of experience in Acute Care. Preferred OCN preferred. Oncology experience preferred. Primary Responsibilities: Assess patient status throughout the episode of care Develop an individualized perianesthesia plan of care by implementing: Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Perform all other duties as assigned . Qualifications Graduate of an Accredited School of Nursing. One year of PACU experience preferred. Current state licensure to practice within good standings. Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 days of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advanced Life Support (PALS) course is required when pediatric care is included in the facility scope services within 90 days of employment or documentation of current PALS certification.
