Duties Description Monitor the conditions of the Individuals in their assigned worksite in order to detect indications of changes in condition or responses to medication or therapy. Duties Description Monitors individual’s medical/health needs and services. Visits each assigned home weekly (minimally) to provide nursing oversight and complete review of items.
