The practitioner leads a multidisciplinary treatment team and oversees the clinical team in the development and implementation of individualized treatment plans. This position collaborates with providers, patients’ families, guardians, and other stakeholders to develop and coordinate comprehensive discharge plans. Responsibilities include prescribing medications; ordering laboratory tests and other diagnostic or therapeutic procedures through the electronic Physician Order Entry (CPOE) system; and completing all required documentation in the Electronic Medical Record (EMR). The role also involves working closely with interdisciplinary team members to coordinate care and treatment, participating in hospital committees as needed, and assisting in the development and revision of hospital policies and procedures when required.
