Duties Description Registered Nurses 2 (Community) are registered professional nurses who provide, oversee, coordinate, and evaluate nursing and health related care and treatment services for individuals* in community settings. Incumbents serve as members of interdisciplinary treatment teams; represent and provide expertise on the nursing aspects of treatment plans and services; and provide nursing services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders and their caretakers or families. PRIMARY DUTIES AND RESPONSIBILITIES: Ensures that care delivered by community providers is consistent with individuals’ overall needs. Visits individuals in community settings to assess care, evaluate individuals’ physical and mental status, and identify factors which may impact their conditions. Observes and records changes in individuals’ responses to treatment efforts and recommends treatment modifications. Completes nursing assessment based on an individual’s health care needs and diagnoses. Makes appropriate referrals to community health care professionals; attends medical appointments; and visits hospitals as required. Provides advice on the availability of medical, social, and housing facilities for the individual in the community. Ensures medical and nursing orders are carried out; monitors quality of care; and evaluates staff performance. Reviews medical documentation, including medication, program participation, and treatment progress. Maintains working relationships and communication with local community health groups. Provides health education and training for individuals, families, providers/employees; and performs appropriate interventions. Administers prescribed medications/treatments and provides health counseling to individuals on their proper use, dosage, and potential side effects. Provides instruction to individuals regarding self-care, self-medication, symptom management, and preventive health strategies. Instructs family members and employees/providers on how to appropriately respond to mental health crises or medical emergencies. Intervenes in crisis situations and administers emergency treatment. Maintains current knowledge and training in cardiopulmonary resuscitation (CPR) and infection control; and instructs individuals and staff on precaution and prevention strategies. May supervise and counsel lower-level staff and participate in facility staff development and teaching programs. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting. Substitution: A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience. Transfer: Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. Per Diem Registered Nurse RN on Acute Care 1 at Lourdes Hospital. Acute Care 1 a 36-bed inpatient at Guthrie Lourdes. This unit provides specialized care for patients with cancer, including active treatment, symptom management, and palliative and hospice services. In addition, the unit cares for patients recovering from strokes, focusing on close neurological monitoring, early mobility, and interdisciplinary rehabilitation. The unit also serves patients with a variety of acute medical conditions such as respiratory illnesses, diabetes, wound care needs, and other acute care diagnoses. The unit utilizes a team nursing model and is known for strong collaboration between RN and LPN teams to deliver safe, patient-centered care across the continuum of illness.
