The On-Call nurse works effectively within RHCD to maintain an environment of excellence which is patient focused and provides timely, appropriate, compassionate, quality client care. Supports the Hospice Philosophy. What you will do Assesses, plans, implements, and evaluates the total nursing care of assigned patients on an intermittent visiting basis. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Assesses client's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or patient visit. Demonstrates assessment ability by applying standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops an initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points. Supervises RNs and LPNs in providing services. Participates in training and support of all team members, volunteers and students as needed. Participates in multi-disciplinary team meetings and discussions regarding quality and outcomes. What you will do Assesses, plans, implements, and evaluates the total nursing care of assigned patients on caseload on an intermittent visiting basis. Applies standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops the initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points. Supervises and delegates non-professional staff to ensure optimum patient care. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems and resources to meet patient care needs. Supervises LPN/LVN's, and Home Health Aides in providing services. Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing. Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Participates in case conferences. Provides education to client and family. Participates in training and support of all team members, volunteers and students as needed.
