Overview Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The On-Call nurse works effectively within RHCD to maintain an environment of excellence which is patient focused and provides timely, appropriate, compassionate, quality client care. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Assesses client's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or patient visit. Demonstrates assessment ability by applying standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops an initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points. Supervises RNs and LPNs in providing services. Participates in training and support of all team members, volunteers and students as needed. Participates in multi-disciplinary team meetings and discussions regarding quality and outcomes. Delegates and prioritizes tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems to meet need. . Bachelor's degree from an accredited college or university in nursing, public health nursing, or other nursing program is preferred.
