Collects delay and other data for specific performance and/or outcome indicators, including but not limited to Avoidable Days, as determined by Director/Manager. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff. Communicates relevant clinical information, including admission review, concurrent review, and discharge planning needs, to third party payors to secure optimal reimbursement. The Case Manager will: Follow 100% of assigned inpatient admissions; Determine the medical appropriateness of the patient’s admission (severity of illness); Monitor the plan of care and proactively identify barriers to the plan (intensity of service); Intervene to avoid delays in plan of care; Identify trends in inappropriate utilization due to delays in diagnostic testing, medical and surgical procedures, and physician management; Coordinate flow of clinical information with third party payors to ensure appropriate reimbursement, proactively identify potential denials in order to avoid non-appealable denials; Credentials: Registered Nurse - NJ (Required) Certified Case Manager (Required) National Case Management certification preferred; or obtained within two years of employment. Overview Emergency Department (ED) Nurse Care Manager is responsible for establishing, coordinating, and maintaining the process to optimize patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum. Overview Emergency Department (ED) Nurse Care Manager is responsible for establishing, coordinating, and maintaining the process to optimize patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum. Supports patient flow: Intervenes to expedite the completion of diagnostic tests needed to determine disposition, offers alternatives to admission when appropriate, engages in boarder rounds to determine any needed changes in disposition. Maintains knowledge of Federal, State, and other regulatory agency rules and regulations : Including The Joint Commission, CMS, Medicare, including but not limited to, knowledge of CMS rules regarding Observation status, the 2 Midnight Rule, and Medicare Inpatient Only Procedures. Maintains knowledge of continuum of cares resources: Provides education and resources relevant to the effective progression of care, utilization of services, appropriate level of care, and safe patient transition to the patient/family and health care team. Advanced Practice Nurse – Care Coordination

To facilitate the discharge process, places rehab orders as needed for the transition of care as required by most post-acute facilities. Facilitates discharge orders including Covid testing orders by communicating with attending physician and writing the actual discharge order. Facilitates the writing of DME (Durable Medical Equipment) orders when needed for patients. Furnish patients with appropriate prescription to obtain needed equipment. Facilitates the writing of level of care orders. When an account does not meet the level of care that the patient was admitted for and with collaboration with the attending physician enter the level of care order in the medical record. With collaboration of the attending physician, writes antibiotic and/or oxygen orders needed for post discharge. Performs peer to peer appeals on post acute denials. Acts as back up for the Physician Advisor handling peer to peer appeals and written appeals when necessary. Performs secondary clinical assessment and documentation to support inpatient admissions as well as to support subacute, long term care(trach weaning necessity/settings) post discharge. Assists denials staff with open communication and feedback to medical staff in regards to denials due to level of care and insufficient documentation. Supports care coordinators to provide real time feedback to providers for improved documentation that will defend the length of stay. Works collaboratively with care coordination manager in process improvement in regards to length of stay, denials and readmissions. Supports care coordination department with difficult discharges and/or refusal to leave facility to a more appropriate level of care. Qualifications Bachelor's degree in Nursing required. Current NJ APN license required. Previous healthcare experience required. Previous experience in an inpatient hospital setting preferred. Experience with EPIC preferred. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
