You will evaluate and manage patients with diabetes and complex endocrine disorders in an ambulatory setting, leveraging standardized protocols, collaborative care planning, and close partnership with endocrinology physicians and the broader interdisciplinary team. Key Responsibilities Provide outpatient evaluation and management for patients with diabetes and a range of endocrine conditions. Co-manage a shared patient panel with endocrinologists in an APP Team Care Model, using standardized protocols and clear escalation pathways. Initiate and titrate endocrine therapies (including insulin and other diabetes medications), interpret labs/diagnostics, and adjust treatment plans. Interpret CGM and glucose data, address complications risk, and coordinate needed screening and follow-up. Educate and coach patients and families on disease management, lifestyle changes, and medication adherence. Coordinate care with interdisciplinary partners (nursing, diabetes education, nutrition, pharmacy, primary care) and support referrals and transitions of care. Document high-quality, compliant clinical notes in the EMR and support clinic access, throughput, and patient experience. Participate in quality improvement initiatives to improve outcomes (e.g., A1c optimization, care-gap closure, workflow standardization).
