Overview JOB SPECIFICS Develop and implement individualized care plans in collaboration with physicians, nurses, and other healthcare providers. Manage medications and treatments to alleviate pain and distressing symptoms. Provide emotional and psychological support to patients and their families. Educate patients and families about illness progression, treatment options, and advance care planning. Coordinate care with primary care providers, specialists, social workers, and therapists. Advocate for patients' needs and ensure holistic, patient-centered care. Excellent work/life balance with weekends. Position will require home visits. Monday to Friday with Call.
