Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) Location: Greenwood Center for Nursing and Rehab | Tamaqua, PA Position: Full-Time Greenwood Center for Nursing and Rehab is seeking an experienced and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to join our skilled nursing facility in Tamaqua, PA. This role is critical to ensuring accurate resident assessments, regulatory compliance, and interdisciplinary coordination in alignment with state requirements and facility standards. Position Summary The RN Assessment Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) and associated documentation to ensure accurate resident assessments, care planning, and compliance with federal and state regulations. Within the urgent care setting care will be provided to occupational health patients (physicals, drug screens, injury care etc.,) and patients that arrive with an acute illness/injury. This individual will work cooperatively with ancillary, nursing and other patient-team personnel to maintain standards for professional nursing practice.
