Collaborates with a physician(s) and other clinicians in providing a full scope of patient care. The focus is on preventive health or the management of acute and/or stable chronic conditions. Performs medical services that have been specifically authorized and directed by the credentialing body and the certifying organization through which they hold licensure. Position Summary The APP provides health care services to patients working in collaboration with a licensed physician member of the medical staff. Makes changes to the care plan as needed and initiates verbal orders and / or collaborates with other visit clinicians. Oversee and supervise the work of Licensed Practical Nurses and other clinicians. LPN Supervisory Visits are performed roughly every 30 days. Submit accurate, complete, timely, meaningful and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing: services, clinical documentation and discussing pertinent patient care issues and goals. Participate in on-call rotation as reasonably assigned. Qualifications Preference for 2+ years in an acute or a post-acute care setting. Active State Registered Nurse licensure is required.
