As an integral part of the interdisciplinary team, the Registered Nurse utilizes the nursing process to provide therapeutic care to patients. The Registered Nurse is accountable for the coordination of patient care and education to the patient and family in accordance with the standards, policies, and practices in a compassionate, supportive, and therapeutic environment. DIAGNOSIS Interprets assessment information and formulates Nursing Diagnoses. PLAN Develops Plans of Care with individualized interventions. Documents expected/projected patient outcomes. Reviews/Revises/Updates Plans of Care daily based on evaluation and interpretation of patient outcomes. INTERVENTION Administers medication and IV therapy appropriately at prescribed intervals. Completes patient care procedures correctly per policy at prescribed intervals. Delegates tasks based on others’ scope of practice and competency. Acts as patient advocate. Requires working in a clinical setting. The noise level is moderate and there is a potential for exposure to infectious diseases and blood-borne pathogens. Job Description The Licensed Vocational Nurse performs a wide variety of patient care activities under the direction of registered nursing personnel. Nursing duties include participating in the planning of patient care, assisting with examinations, procedures and treatments, administering medications within scope of practice, and following medication management per policy. Duties also include assisting with ADL's, taking of patients' vital signs, and recording results on patients' charts. Responsibilities Administers patient medications, including basic IV management, within scope of practice, and participates in treatments and procedures in a safe, effective and age appropriate manner. Recognizes changes in patient conditions, taking appropriate action and documenting intervention. Uses knowledge of aseptic techniques in performance of treatments, such as sterile dressing, irrigation, and cauterizations. Effectively collaborates with the health care team to intervene, and advocate for the safety of the patient, as necessary. Assists patient with hygiene, elimination, routine skin care, comfort measures and other Activities of Daily Living (ADL), in consideration of patient age, gender, culture and stated preferences, maintaining patient privacy and respect at all times. Maintains a clean & safe environment. Ensures Infection Control procedures and HIPAA compliance. Performs assigned nursing duties for the safety, comfort and well-being of patients. Accurately measures and records vital signs. Assists with patient care activities including specimen collection, turning and positioning, and patient transportation. Certified Nursing Assistants lend support to the Licensed Vocational Nurses in the medial necessities of all residents. ESSENTIAL FUNCTIONS Actively support the basic rights of all residents. Perform skills required of a nursing assistant related to provision of basic ADL care, provision of nutrition and hydration, toileting program, incontinent care, vital signs, and other care as needed. Assists residents with linens, bed making, dish washing, trash, and laundry as needed. Assist residents with transportation to meals, activities, beauty shop, and medical appointments. Supervise and account for residents in dining room. Assist residents to make menu selections. Monitor residents for problems related to meals and food intake, report problems. Assist residents with cleanliness, grooming, and nourishment needs. Ensure safety of resident by maintaining a safe environment and using appropriate behavioral intervention techniques. Assist resident’s self-administration of medication as needed. Document medication assistance provided. Report problems or refusal of medications to RCFE Health Service Manager. Provide support and guidance to residents for blood sugar monitoring and other assistance with adaptive equipment or basic care. Complete all required documentation related to centrally stored medications, medication refills, etc. Reorder medications from pharmacy. Maintain awareness of resident health. Inform appropriate staff (LVN, RCFE Health Service Manager) of any incidents or changes in resident condition, such as attitude, appearance, appetite, behavior and/or physical ability. Obtain Vital Signs, document and report unusual readings to LVN. Document resident care, behavior and special needs as required by policy and procedure. Attend scheduled staff or campus meetings, in-services, and events. Ensure that all residents’ rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights including the right to wage a complaint, are well established and maintained at all times. Always preserve the dignity and self-respect of the residents. Perform other related functions as requested by the LVN or RCFE Health Service Manager.
