ESSENTIAL FUNCTIONS Lead team of Per Diem RN Coordinators Supervises RN Coordinator activities Ensures all relevant high-risk data is communicated to nursing staff before assessments Schedules initial assessment and reassessment/supervisory visits Conduct field orientation of newly hired RN Coordinators Annual or semiannual reviews of the per diem RN Coordinators' performance SUPERVISORY ACTIVITIES Assists and conducts ongoing in-service education and compliance training of caregivers Monitors caregiver job performance through skills assessment evaluations and in-home supervisory visits CARE MANAGEMENT Coordinates client services by reviewing plans of care and frequency of care Engage with clients/relatives who are reported to have CICs (change in condition) by the caregiver and decide if an RN visit is necessary (before the 90-day schedule) Other essential functions Ensures compliance with company policy and all federal, state, and local regulatory bodies and conditions of participation Remains informed and updated on home health regulations and standards Offers suggestions in the development and implementation of policies and procedures related to RN Coordinator activities. As the Per Diem Registered Nurse (RN) Coordinator , the goal is to ensure clients receive the highest quality care in accordance with Company standards. Within the framework of the objectives and policies established by the Company, plans and coordinates care objectives that promote quality within the Company both within and outside the Company.
